Old Media Flounders with Ohio U.S. Representative Stephanie Tubbs Jones Death Reports

While the Associated Press refers to this SNAFU with quotes calling for soul searching, this is a concertedly bizarre accounting of things.
How exactly do news outlets explain their reliance on “anonymous source,” “on background,” information about a Congresswoman’s death - when the story turns out to be dead wrong? Better yet, how do “Democratic officials speaking on condition of anonymity” explain that they knew the Congresswoman had passed away, prior to her actual passing?
Condolences to the family, and peace to Mrs. Jones.
